New Haven, Conn. (WTNH) – The Bridgeport Hospital Primary Care Center helps administer a Patient Centered Medical Home program in the Bridgeport area. The program uses a team-based approach, where physicians and other health care providers work hand-in-hand with Team Health Care Coordinators, Patient Care Navigators, Social Workers and pharmacists.
